Medicare/Medicaid Integration Program
The Centers for Medicare and Medicaid Services (CMS) estimates that there are 6.2 million people in the U.S. that are eligible for both Medicare and Medicaid. Many in this group have complex medical and chronic care needs that require lengthy stays in a variety of long-term settings. Effective care management for this population can best be accomplished when health plans can coordinate the entire continuum of health and long-term care services. Current Medicare and Medicaid policies contain many obstacles to integration. The purpose of the Medicare/Medicaid Integration Program is to encourage states to integrate Medicaid's long-term care services with Medicare's acute care services through managed care.
The program is sponsored by The Robert Wood Johnson Foundation(RWJF). The program has funded 14 states that are in various stages of planning, development, and implementation. Three of these programs, Colorado, Florida, and Washington, have completed their funding cycle.
