Certificates of Insurance

Mason’s Office of Risk Management (ORM) will provide Certificates of Insurance (COI) on behalf of the Commonwealth to demonstrate adequate insurance coverage.
COI request forms can be found on ORM’s website at risk.gmu.edu for both outside organizations and inter-department purposes.
Submit requests five business days in advance of when the certificate is needed. The contract or agreement must accompany the request for COI. Mason cannot agree to any additionally insured, indemnification, or hold harmless provisions.
All contracts and agreements must be signed by Mason employees who have expressly been delegated signature authority by an official Memo of Delegation of Signature Authority. Otherwise, no employee has legal authority to bind the university.
