For the safety of our community, Mason will require vaccines for all students, faculty, and staff, and to share verification of their vaccination status to work, study, and live on campus.
Mason has adopted University Policy 1416 on COVID-19 vaccinations. The university will hold no-cost vaccination clinics through the fall semester. Schedule an appointment to get your shot.
All students were required to be fully vaccinated by August 1, 2021, unless they received an approved exemption.
You should also have uploaded your vaccine documentation to the Health Services Patient Portal by August 1, 2021.
Even though the deadline has passed, you should still upload your documentation. Students must meet the requirements in university policy 1416.
If you're an international student with limited access to COVID vaccines, contact Mason to request a deadline extension. Students with approved extensions must be vaccinated once they arrive in the United States.
Mason offers flexible excused absence options for students receiving vaccination and those with side effects after vaccination. This includes international students whose in-person attendance may be delayed due to CDC international travel quarantine requirements.

Faculty and Staff
- Inform the university of your vaccination status through the daily Mason COVID Health Check.
- Upload your vaccine documentation into the Health Services Patient Portal. If you’ve gotten at least the first dose by August 15, upload your vaccine document. When you get the second dose, upload the completed document.
- To request a medical exemption, fill out the Medical Exemption form (PDF), which must be completed and signed by a licensed medical provider. It must also include a medical diagnosis. Upload the completed form, following these guidelines, through Mason’s Patient Portal.
- To request a religious exemption, fill out the Religious Exemption form (PDF), which must be signed in front of a Notary Public and include a Notary Public Seal. Bring the form to the Johnson Center Testing and Vaccination Clinic on the Fairfax Campus (Monday to Friday, 7 a.m. to 3 p.m.) or mail the form to:

If you’ve lost or damaged your vaccine document, you can request a replacement by emailing the Virginia Immunization Information System help desk or calling 1-866-375-9795 to request a replacement. The Help Desk is staffed 8:30 a.m. to 5 p.m., Monday through Friday. You can also fill out an Immunization Record Request Form.
Need a Vaccination?
- You can get an appointment at Mason’s Fairfax Campus, often on the same day, through our Vaccine website.
- If you can’t make it to the Fairfax Campus, the government vaccine website lists locations to receive a shot.
- Mason employees have an allotted 8 hours of Public Health Emergency Leave to get the vaccine and recover from any side effects.
If you start work at Mason after August 15, 2021, you must:
- Have received at least one COVID-19 vaccination by your start date, be fully vaccinated within 45 days of your start date, and submit documentary proof through Mason’s Patient Portal, or;
- Have an approved medical or religious exemption by your start date. To request an exemption, fill out the Religious Exemption form (PDF) or the Medical Exemption (PDF) form. Follow the directions listed under employee requirements.
Remote Work Exemptions
Employees who have initiated or plan to initiate 100% remote work agreements, and who pledge as part of this agreement that they will under no circumstances come to campus, may also be issued an exemption. Remote work agreements are subject to approval by your supervisor and must also be reviewed by your unit or department head.
If you have an approved Full-Time In-State or Out-of-State Telework Agreement, don't intend to come to campus, and are requesting an exemption from the university's COVID-19 vaccine requirement, please fill out the Employee Agreement Regarding Campus and University Activities form.
