International Emergency Travel Insurance

Travelers requiring a Declaration of Coverage letter for use during the visa application process should complete the request form and submit it to the Office of Risk Management at least two weeks prior to their visa application appointment. 

Faculty and staff traveling on international business should complete the Insurance Registration Form at least two week prior to travel.

Insurance ID cards and coverage information will be provided following receipt of the completed form. 

Take note of holidays and university closures, such as winter break, as requests will not be processed while the university is closed.
